It may also be used as a migraine preventative medication or for pain management (Whalen, Feild, & Radhakrishnan, 2019). Amitriptyline is absorbed and metabolized within the liver with a varying level of first pass metabolism. Amitriptyline joins with glucuronic acid before moving to the kidneys, where an inactive form is excreted through the urine.

There are several side effects which have contributed to the recommendation to avoid Amitriptyline in the elderly population. Some of these adverse effects include orthostatic hypotension, tachycardia, sedation and lightheadedness. Given the potential safety concerns that would accompany these side effects, it is recommended for avoidance on the Beers Criteria. If an elderly patient were to utilize Amitriptyline as a part of their treatment, dosage adjustment may be needed to take into account liver concerns that can occur with aging, thus affecting absorption and metabolism rates.
