I will complete my hours in diabetic clinic (Cleveland Clinic). This is an excellent setting for me to learn more about diabetes and how to effectively treat patients with the condition. The hospital offers crisis intervention, counseling, and support services to diabetic patients and their families (Lowe et al., 2021). I am excited to have the opportunity to learn from experienced social workers and to provide support to diabetic patients. I am looking forward to gaining more experience in this setting and learning from the nurses and doctors who work there.
A Cleveland Clinic diabetic clinic is a specialized clinic that provides care for people with diabetes (Pantalone et al., 2018). The advanced public health nurse plays an important role in these clinics by helping to coordinate care for patients, providing education and support to patients and their families, and working with other members of the healthcare team to ensure that each patient receives the best possible care. The advanced public health nurse works with patients to create a personalized treatment plan, helps them to understand their condition and how to manage it, and provides ongoing support and education (Schmitz et al., 2019). In addition, the nurse assists with screening for diabetes-related complications, coordinates care among specialists, and helps patients to make healthy lifestyle choices.
One of the overall clinical/practicum goals selected is to seek guidance from a specialist in educating my patients about the disease process and best practices for disease management over the school year. Every six months, I will assess for an improvement in uncontrolled diabetes cases on the campus. Another goal is to review guidelines and policy changes that affect health management in schools and make necessary adjustments completed by the first quarter of the year. The selected goals and the characteristics of the clinical setting align to ANA Public Health Nursing Standards. ANA Public Health Nursing Standards require all nurses to understand diseases processes and best practices for disease management, these factors are well captured in the practicum objectives as well as the clinic/setting where the practicum activities will be undertaken.
