Healthcare aggression and violence refer to incidents where healthcare professionals are threatened, abused, or assaulted in their line of duty. Aggression is any behavior by a person or people within or outside the organization to physically or psychologically harm an employee. Workplace aggression occurs in physical and non-physical forms. Physical aggression entails using force against an employee resulting in physical and psychological harm like kicking, beating, or slapping (Vincent-Höper et al., 2020). Non-physical aggression often occurs verbally and comprises all types of verbal abuse, like intimidation, insults, swearing, and offensive language. Workplace violence is a precise type of workplace aggression that includes behaviors aimed at causing physical harm.
Aggression and violence in healthcare settings have become a growing public health concern in the US and other countries. Nurses are the healthcare professionals at the highest risk of encountering aggression and violence. Vincent-Höper et al. (2020) assert that nurses in emergency rooms, psychiatric settings, geriatric care settings, and hospitals report the highest incident rates. Healthcare aggression and violence cause excessive socio-emotional stress, resulting in psychological strain. Nurses and other healthcare providers often face violence and aggression, potentially impairing their physical and mental well-being. Studies show that exposure to aggression and violence impairs healthcare providers’ mental health, as evidenced by signs of burnout, depression, and post-traumatic stress disorder (Vincent-Höper et al., 2020). The consequences are usually severe to the providers and the patients they serve. For instance, it adversely affects the quality of care provided by the providers subjected to aggression due to absenteeism and deterioration of the working climate.
The middle-range theories most valuable in addressing the aggression and violence issue are Nurse as Wounded Healer theory and A Middle-Range Theory of Nurses’ Psychological Trauma. Conti-O’Hare’s theory of a Nurse as Wounded Healer asserts that the nurse is positioned as both a wounded person and a healer. The theory emphasizes and prioritizes the nurse as a healer and integrates the nurse’s trauma into the role of a healer (Piredda et al., 2022). Besides, it frames the model of the nurse as a wounded healer from the existence of old wounds that enable the nurse to improve patient care and competence. Conti-O’Hare’s theory can be used in events of aggression and violence to address personal and professional pain, develop therapeutic relationships, and promote positive working environments for health providers (Piredda et al., 2022). For example, nurses can apply the theory to gain insights and learn from past experiences of aggression, which can be used to help others as they face pain and suffering.
The Middle-Range Theory of Nurses’ Psychological Trauma states that traumas can be nurse-focused or patient-focused. Each person perceives trauma differently because the incidents, experiences, and effects of the traumatic event are unique to the person. The theory asserts that traumas like workplace violence can be mitigated or prevented by the environments generated by systems and organizations (Foli, 2022). Thus, the theory can be used to implement trauma-informed self-care to reduce the severity of individual nurse-specific traumas. For example, the theory can be used to identify healthcare systems and organizational strategies that provide safe working conditions to nurses to reduce the severity of nurse-specific psychological traumas.
