The patient’s health needs include vasomotor symptoms like hot flushes and night sweats, which can be associated with perimenopause since the patient is transitioning to menopause. She has genitourinary symptoms that can also be attributed to perimenopause. The patient also has blood pressure management needs owing to her history of HTN and an elevated BP of 150/90.  Besides, she has weight management needs considering her current weight of 230 lb. The patient has previously had abnormal cells on Pap smear, which increases her risk of cervical cancer, while the family history of breast cancer increases her risk of breast cancer.

The recommended treatment therapy will be hormonal replacement therapy (HRT), comprising a combination of estrogen and progesterone.  According to Palacios et al. (2019), estrogen is the primary active component of HRT and is the gold standard for managing perimenopausal symptoms, especially vasomotor symptoms. Bansal and Aggarwal (2019) assert that females with an intact uterus need progestins alongside estrogens to prevent endometrial hyperplasia and carcinoma. Therefore, I will recommend Estradiol 25 mcg transdermal patch as the estrogen replacement and Micronized progestin 100 mg. I recommend estrogen transdermal patches since they have a lower risk of vaginal bleeding, breast tenderness, thromboembolism, and stroke. Furthermore, I would continue Norvasc 10 mg OD and HCTZ 25 mg to maintain an optimal BP.

An example of a health education strategy that can be utilized with this patient is providing health educational materials.  This includes pamphlets, YouTube video links, and podcast links on the management of perimenopausal symptoms. The educational materials should also include education on lifestyle interventions to promote weight loss and maintain optimal BP. Bhattad and Pacifico (2022) explain that patient education materials help to educate patients on their health conditions, improve their health literacy, and foster informed decision-making based on the most current and updated medical and clinical evidence and patient preference.
