Presentation: Wendy Chang is a 61-year-old female with Multiple Sclerosis, Diabetes type 2 and mild hypertension treated with medication. Wendy recently had a relapse of her MS and end up in hospital not being able to walk, had slurred speech, vision impairments, urinary incontinence and problems showering, preparing food and eating. She said at the time she felt extremely fatigued and emotional. She thinks the relapse happened because she got too hot out gardening on the day and her blood sugar levels have been elevated to 12.6mmol to 14.5mmol on some of the readings for the last few days. Wendy was diagnosed with Relapsing-remitting MS as this was her third presentation.
Treatment in hospital: Wendy is normally independent living at home with occasional falls. In the hospital, she had a PICC inserted for IV fluids and a course of Methylprednisolone IV for a few days and this was successful in reversing some of her symptoms and for rehydrating her. She has worked with the physiotherapist for her mobility and is safe to go home with using a 4-wheeled walker.
Her daily prescribed medications are as follows:
Metformin 1gram BD
Panadol 1 gm TDS
Baclofen 10mg TDS
Methylphenidate (Ritalin- permission given for atypical use in MS) 20mg Daily
Zoloft 50mg daily
Atacand 8mg BD
Aspirin 100mg Daily
Discharge planning: The doctor said she can go home if she has safety measures in place and has carers come to her home to assist her when she showers, for preparing meals and to go out shopping etc. She will need help to attend appointments and to clean her house. She still has significant spasms and has numb feet but is walking again with her aid. Wendy usually lives independently, and enjoys yoga and morning coffee outings with friends and going to the movies. She tries to stay active by walking early in the morning when it is cooler. Her husband passed away 4 years ago from a car accident but her two adult daughters live close by but are busy with their careers. She occasionally experiences depression associated with grief and the loss of her husband and her own functionality.
The neurologist wants to leave the PICC inserted as they may need to do more treatment in a few weeks and will be reviewing her in two weeks to see if she declines further with her illness or if she continues to make improvements. The specialist thinks they may need to undertake plasmapheresis in the next few weeks if Wendy’s condition relapses quickly again. They are also considering starting Wendy on disease-modifying drugs, but this is in early planning and consultations are needed with the Professor of Neurology in Melbourne. The decision will be made at her follow-up appointment in 2 weeks.
You are planning Wendy’s discharge home and have been successful in securing a home care package for Wendy with carers coming in twice a day for two hours per visit under the Transitional care program for acute hospital presentations returning home. This care agreement lasts for 2 months until Wendy can get rehabilitated or get a disability support package. You will need to plan a self-management plan with Wendy so that she can selfmanage her carers and her immediate care needs.
