The case scenario concerns a 46-year-old woman presenting with RUQ pain that has lasted 24 hours. The pain started an hour after having dinner, and she had nausea and one episode of vomiting before the onset of the pain. Findings include a non-distended abdomen but with mild tenderness and high WBC and Direct bilirubin levels. The purpose of this paper is to explain the diagnosis and treatment of this patient.

Diagnosis

Hepatitis A is the likely diagnosis for this patient. Hepatitis A is an infectious disease caused by hepatitis A virus (HAV), which is spread through the fecal-oral route through consumption of HAV-contaminated food and water and person-to-person contact. According to Abutaleb and Kottilil (2020), the clinical manifestations of Hepatitis A include anorexia, jaundice, abdominal pain in the right upper quadrant, and elevated bilirubin levels. The patient presents with symptoms consistent with Hepatitis A, including nausea, vomiting, abdominal tenderness, RUQ pain, and increased direct bilirubin levels. The high WBC count points to an infection.

Drug Therapy Plan

Supportive therapy is the mainstay treatment for Hepatitis A. The patient’s drug therapy will include Plasil 10 mg PRN and Tylenol 500 mg PO BD. Plasil is an antiemetic and will be prescribed to relieve nausea and vomiting (Webb et al., 2020). Tylenol is an analgesic and will be prescribed for pain relief to help alleviate the abdominal pain. Inactivated Hepatitis A vaccine will be recommended to actively immunize the patient against illnesses caused by HAV (Ntouva et al., 2019).

Conclusion

Hepatitis A is the presumptive diagnosis owing to the patient’s positve symptoms of nausea, abdominal tenderness, RUQ pain and elevated direct bilirubin and WBC levels. The patient might have contracted the Hepatitis A virus from consuming infected food and water. Supportive treatment will be used to alleviate nausea/vomiting and abdominal pain using Plasil and Tylenol.
