Triple Aim Initiative

Most healthcare organizations have been formulating systems that can optimize healthcare performances, such as Triple Aim. According to Gesko et al. (2020), Institute for Healthcare Improvement (IHI) developed Triple Aim to help healthcare leaders manage the health of their target communities. IHI’s Triple Aim consists of three principles beneficial to long-term or acute care settings necessary for patient satisfaction, safety, and quality care improvement. These principles include patient experience, per capita cost control and reduction, and population health.

Patient’s Experience

In the patient’s experience, the main factors regard identifying community needs and overall health. Gesko et al. (2020) accentuate that through IHI’s Triple Aim, healthcare systems can improve how they offer patients’ experiences towards their satisfaction, safety and enhancing healthcare service quality. In such a case, more effective initiatives on patient experiences are needed by the healthcare leaders. As indicated by Gesko et al. (2020), some of these initiatives include the use of advanced information technology applications like electronic health records or telehealth that enhance effective interactions and communication among patients and healthcare professionals. On the same, there will be a need to track patient experience where measures like patient satisfaction surveys will help assess patients’ attitudes and perception towards the changes.  In such a case, patient experience enhancement will improve patient satisfaction in the long-term or acute care setting.

Per Capita Cost Control and Reduction

Most patient communities tend to seek affordable and quality from different healthcare organizations, especially those who need consistent care. In that case, Gesko et al. (2020) assert that IHI’s Triple Aim helps organizations provide affordable care in long-term and acute care settings for patient satisfaction. The principle is essential since it allows organizations to incorporate various systems initiatives to help control healthcare costs and minimize funds mismanagement to increase service quality.  In that case, the most appropriate measure compatible with IHI’s Triple Aim for healthcare services cost reduction is the use of electronic health records (Gesko et al., 2020).

For instance, if a long-term and acute care setting uses electronic healthcare records, they will manage to save costs that they would undergo daily, especially in employing more than needed members of staff. Also, through electronic health records, the hospitals become assured of increased practice efficiencies since the healthcare professional can attend to more patients in a day. Through mobile technologies like mHealth applications, healthcare facilities will reduce inefficiencies as they deliver care in a long-term and acute care setting.  That is because the mHealth applications work towards promoting cost-effective measures like self-care management for the patients. For instance, patients who have been managing chronic diseases like diabetes in a long-term care facility can be trained on how to seek help using mHealth applications as a strategy to monitor their health and access healthcare professionals easily.

Population Health

The third IHI initiative principle regards population health. In that regard, Gesko et al. (2020) notify that Triple Aim helps healthcare organizations to establish measures that will help promote their target population health. In such a case, it will be necessary for healthcare facilities first to identify and seek means to address the target population’s risk. The main reason those factors are worth assessing is the high possibility of each community’s individual becoming a patient. Hence, it will be advisable for long-term or acute care facilities to embrace the principle. These healthcare facilities can integrate the principle by involving the target communities in designing their care models and enhancing patient healthcare education. Such an approach will increase instances of disease prevention and in promoting community health.

Healthcare Future Roles Reflection

Directors of nursing major role regard executing various healthcare responsibilities on nursing practice. Reinking (2020) asserts that directors of nursing coordinate and manages the nursing personnel services. Also, these directors of nursing are responsible for reviewing and supervising the nursing staff, managing patients’ medical records and data, interacting with families, patients, and doctors, and overseeing departmental budgets. Such responsibilities are necessary since directors of nursing helps to observe patient safety and satisfaction and oversee healthcare cost and quality offered in a healthcare facility.

In that regard, when I assume the role of a director of nursing, there are various measures that I will put into considerations to ensure that I meet patients’ safety and satisfaction and in the provision of affordable quality care. For instance, I will assess cases in which patients have been subjected to overtreatment. The reason for that approach is due to the harmful and expensive nature that overtreatment poses to patients. Generally, overtreatment occurs due to ignorance among some careless nurses that can destroy a reputable brand of a healthcare facility (Zaheer et al., 2019). Hence, to ensure that the healthcare facilities’ brand reputation in offering quality, safe, and quality services stands out, I will schedule regular sessions with the physicians and nursing staff. In these regular sessions, I will be informing them on various nursing practices that they should avoid, such as basing the treatment on the assumption rather than carrying out evidence-based research for safer treatment.

My role as a nursing director will mainly rely on transformational leadership, which poses a lot of relevance in healthcare and nursing. According to Ferreira et al. (2020), transformational leadership brings a long mediating spirit between leaders and their employees, thus enhancing collaboration. Hence, despite my position in leadership, I will need to establish a positive practice environment where I will need to offer encouragement, carry individual consideration and conduct open communication to enhance relationships with the healthcare providers.  I am sure that creating such opportunities to allow meaningful conversations and dialogues with the healthcare nurses will promote patient safety and satisfaction. Hence, despite the practice seemingly complex currently due to high-paced managerial roles, nurses need support from their leaders, which will be essential towards better patient outcome achievements.

Reducing Medical Errors for Frontline Nurses

Medical errors pose serious health problems to the public, which jeopardizes healthcare organizations’ role in providing a safe environment to the patients. The action that I will take as a director of nursing includes putting more effective measures in reducing clinical errors. According to Zaheer et al. (2019), nursing directors should ensure that they reduce instances that risk a patient’s health and well-being within a healthcare facility. Generally, some mistakes that healthcare providers pose a significant risk to patients and become more expensive. Hence, to improve medical error instances towards achieving patient safety, the main factors in evaluating are for the nurses to double-check medication, seek help, proper documentation keeping.

Double-Check Medication

It becomes hard to eliminate nursing practice errors at times, thus requiring a need to enforce some measurable practices. According to Zaheer et al. (2019), the best way that directors of nursing can prevent medical errors include recognizing the medical error instances, learning from them, and taking preventative actions. In that case, some of the measurable practices that I will encourage are checking and double-checking the medication since it is easy to lose track of how to medicate the patients. Such measures will help the nursing staff and physicians to avoid making medication errors.

Seeking Help

Another factor that I will use to minimize nursing errors among the nursing practitioners and physicians regards encouraging them to seek help. In most cases, one will find that some nurses may offer wrong medication to the patients knowingly due to fear of consulting (Reinking, 2020). Hence, training and encouraging the nursing practitioners to always seek help without feeling ashamed will improve the nurses’ relationship. That is because consultation’s main goal will provide safe, quality healthcare services by preventing the patients from incurring another cost seeking wrong medication treatment. Hence, as a director of nursing, I will ensure that I establish and maintain a culture that helps the nursing staffs recognize safety challenges and implement solutions instead of maintaining a culture that punishes, blames, and shames healthcare staff. Hence, involving a transformational leadership style will make it easier to commit the nursing staff towards providing a better environment to practice patient safety and satisfaction services.

Proper Documentation

The main reason why most nurses experience medical errors is through failing to keep proper documentation. Hence, it will be necessary to enlighten the frontline nurses on keeping all documents to avoid wrong medication. One way to do so is by educating the nursing staff concerning the two different medical errors. Sohrevardi et al. (2017) posits that the primary medical errors include;

Errors of Omission

The error comes along when the nursing professionals do not take any action. For instance, when dealing with patients that need wheelchairs to move around, the nursing professional might omit to strap the patient who might cause other safety issues such as the patient falling off and breaking their legs or hands.

Errors of Commission

Medical errors of commission occur when the nursing professionals take the wrong action. An example of a medical error of commission regards medicating a patient with medicines that will cause more harm to their bodies, such as administering medications that become allergic to a patient.

Conclusion

The reflection projects the aims of IHI’s Triple Aim initiatives and effectiveness in observing patients’ safety, satisfaction, and improving healthcare quality. Also, other factors included in the reflection regard my future healthcare roles and reducing medical errors for frontline nurses. These factors are essential since they help offer quality services to patients, especially those in long-term and acute care settings.

Healthcare Policy and Economics
Module 4- Assignment
Overview
The Institute for Healthcare Improvement (IHI) Triple Aim Initiative serves to change health care delivery by (1) improving the patients’ experience of care; (2) improving the health of populations; and (3) reducing the per capita cost of health care (Nickitas, Middaugh, & Feeg, 2019). This week’s module explores how we can improve the patient care experience, increase the quality of care, reduce the cost of care thereby improving the health of the populations. In addition, we will examine the concept of implementing patient/person-centered care (PCC) in our current healthcare practice. The role of the Joint Commission and other regulatory agencies, and patient and healthcare worker safety will be a major focus. Quality measurements such as patient satisfaction surveys and error control mechanisms will be explored.
