Assignment: Diagnostic Safety

Diagnostic safety is defined by timely and accurate diagnosis. This is crucial to effective clinical practice and a vital initial step to attaining optimal patient outcomes. Diagnosis usually involves a series of events across different dates, healthcare settings, and providers and often evolves over the course of a patient’s care (Singh et al., 2019). Consequently, these events may cause errors in the diagnostic process linked to numerous factors, including healthcare professionals, patients, and complex healthcare technology systems.

Diagnostic safety is evidenced by the lack of diagnostic errors. A diagnostic error is defined as the failure to make an accurate and timely explanation of a patient’s health problem or communicate this explanation to the patient. It is a diagnosis that is wrong, missed, or delayed. Diagnostic errors contribute to significant patient harm, increased healthcare costs, and unnecessary healthcare utilization. They are a significant healthcare concern since they affect every patient at least once in our lifetime and can have distressing consequences (Abimanyi-Ochom et al., 209). Furthermore, diagnostic errors account for the greatest percentage of malpractice claims, the most adverse patient harm, and the greatest proportion of penalties to providers and hospitals.

Diagnostic safety is compromised when the initial diagnosis is established as inaccurate after the accurate cause of the patient’s clinical symptoms is identified later, which is regarded as a wrong diagnosis. It is also compromised when there is a failure to timely diagnose a disease, leading to a delayed diagnosis, especially in cancer cases (Abimanyi-Ochom et al., 2019). Furthermore, diagnostic safety can be compromised when a patient’s complaints are not clarified, and specific complaints fail to be correctly diagnosed, termed a missed diagnosis. Improving the accuracy and appropriateness of diagnoses can promote diagnostic safety and, as a result, reduce costs from inappropriate diagnostic tests, incorrect treatments, and malpractice lawsuits.
