The available data indicated a 20% increase in the cases of operations to the wrong site and patient within one year in the surgical unit. The process of obtaining this data was faced with some challenges. First, nurses were reluctant to provide the actual number of cases of surgeries on the wrong patient and the wrong site in the surgical unit. Providing the actual figures would indicate the provision of poor quality and unsafe patient care. By so doing the medical facility would be acting in contrary to the quality standards set by organizations such as the Joint Commission on Accreditation of Healthcare Organizations (HCAHO). Secondly, data about some cases of surgeries on the wrong patient and the wrong site in the surgical unit was not available since it was not documented. Some nurses fail to document the major mistakes that they make during care delivery. According to Krishna and Khyati (2017), improper documentation limits communication between a healthcare organization and the outsiders.

The cases of surgeries on the wrong patient and the wrong site in the surgical unit are measured by comparing the actual cases and the quality standards set by organizations such as HCAHO. Deviation from these standards indicates compromised care.

Gaps in the Data and Additional Sources

            Some gaps were identified in the data about cases of surgeries on the wrong patient and the wrong site in the surgical unit. First, the data did not indicate the patient population at a high risk of this clinical problem. Lack of this data hinders the efforts to improve the quality of care among the most affected population. Additionally, the data did not include the primary causes of this medical error. This problem is relevant to the nursing practice since it reflects the quality of care provided to the patients. Addressing this clinical problem is substantial to the nursing practice since it will improve the clinical outcomes resulting in patient satisfaction.
