Integrating Evidence-Based Practice

The primary goal of any medical organization is to engage in effective and efficient treatment of any health problems to be in a position to provide high-quality, individualized, and patient-centered care. From such goals, the healthcare industry has identified various protocols and methods to incorporate into the healthcare services evidence-based practice. Evidence-based practice (EBP) is an important initiative in the healthcare industry since it helps healthcare providers make informed medical decisions through the best available research information. In addition, EBP is acknowledged as the most efficient way to generate answers to particular medical issues in the medical realm since it helps evaluate different options before making any clinical decision. Therefore, there is a need to ensure that all healthcare providers understand what evidence-based practice entails and its role in the healthcare industry. The paper will consist of two parts; part one will involve assessing eight steps to integrating evidence-based practice, barriers in implementing the new practice, and strategies to overcome these barriers. Part two will integrate a description of six internal evidence sources necessary to provide data for outcome improvement.

Assessing Eight Steps to Integrating Evidence-Based Practice

According to Melnyk and Fineout-Overholt (2015), the main steps for evidence-based research include establishing a formal implementation team, building excitement, project dissemination, clinical tools development, evidence-based practice change, and preserving the energy sources timeline, and celebrating the success. The research under study involves evaluating the effectiveness of CPAP therapy in reducing the risk of high blood pressure.

Establishing Formal Implementation Teams

The formal implementation of teams is a step that should be taken once the researcher has formulated the PICOT question. According to Melnyk and Fineout-Overholt (2015, p. 212), the step should help the researcher outline all individuals to make the research successful. In that case, the main implementation in our clinical setting will involve Advanced Practice Registered Nurses (APRNs). They are essential for the change theory since they function at the systems level. Nursing staff members will also be essential since they will assist in ensuring that CPAP therapy is clinically applicable and feasible for smooth adoption by the healthcare organization. There will also be a need to engage the community, patients, and their families as part of the opinion leaders that can evaluate the effectiveness of the planned change.

Building Excitement

As Melnyk and Fineout-Overholt (2015, p.212) indicate, in the building excitement phase, the researcher explains how to ensure that all stakeholders support the change. Hence, there will be a need to engage all stakeholders, including the nursing staff, in a general meeting to brief them about the proposed project. In addition, there will be a need to engage concise evidence on how CPAP therapy has led to positive results in other healthcare organizations.

Disseminating Evidence

The step will mainly aim at engaging all those that need to be part of the proposed change (Melnyk and Fineout-Overholt, 2015, p.212). In that case, there will be a need to engage the stakeholders, especially the nurses in the workshop, to train and increase their knowledge of CPAP therapy. Hence, the main experts to address the nurses in these workshops include a cardiologist.

Developing Clinical Tools

According to Melnyk and  Fineout-Overholt (2015, p.212-213), evaluating the clinical tools essential for EBP research is always necessary. Some factors to consider are patient documentation and resources like EBP summaries necessary to make the new practice successful.

EBP Change Pilot Test

There is always a need to ensure that new change is assessed before it can be put into full practice (Melnyk & Fineout-Overholt, 2015, p.214). Hence, in assessing the effectiveness of CPAP therapy, the research team can engage a sample of patients with high blood pressure and take their review the new change. The best way to evaluate the change effectiveness is by providing the pilot test group with survey questions to rate the CPAP therapy.

Preserving Energy Sources

There is a need for the research leader to ensure that the implementation team is not stressed for better change implementation outcomes (Melnyk & Fineout-Overholt, 2015, p.214). The best way to preserve the project team’s energy is by giving them enough time to rest to rejuvenate to keep them productive.

Success Timeline

There will be a need to engage a success timeline to ensure that the change project team will use that timeline to complete tasks and use the project budget sparingly (Melnyk & Fineout-Overholt, 2015, p.215). Hence, it is important to engage in a continuous evaluation and monitoring of the outcomes changes.

Success Celebration

As Melnyk and Fineout-Overholt (2015, p.215) indicate, the last step also involves acknowledging the input of every member involved in the EBP practice, such as the clinicians, healthcare department leaders, and nurses.

Barriers to Implementing the New Practice and Ways to Overcome these Barriers

The main barriers to implementing any new practice include nurses’ limited knowledge of applying EBP, staff shortage, and heavy patient caseload. In addition, according to Alatawi et al. (2020, p.38), nurses do not acknowledge how they should incorporate EBP in their practices, which affects their attitudes towards it. For instance, most nurses may fail to acknowledge the effectiveness of CPAP therapy compared to oral medications such as the administration of calcium channel blockers or diuretics. As a result, most end up evading EBP inclusion as they continue to provide healthcare services resulting in poor decision making, thus influencing patients’ outcomes. However, to curb such barriers, healthcare organizations need to engage these nurses in workshops and training to enhance their EBP skills to help them embrace its utilization in nursing practice. Also, healthcare institutions should make EBP compulsory in the nursing curriculum, which will help nursing students acknowledge its effectiveness before transitioning to nursing practice.

Furthermore, the issue of staff shortage and heavy patient load is another factor that affects the implementation of a new change since nurses may fail to have the necessary resources to implement the changes (Alatawi et al., 2020, p.39). For instance, a shortage of nurses with an increase in patient overload may hinder nurses from identifying crucial factors like the major causes of high blood pressure in various demographics and end up relying on the existing research. The main way to solve such issues is for healthcare facilities to engage technology such as telemedicine and telehealth, which can help nurses complement the patient-nurse ratio. Such an approach will also help the EBP researchers have a variety of information that can improve healthcare outcomes for a larger patient base.

Part Two

Description of Six Internal Evidence Sources for Healthcare Outcome Improvement

Healthcare outcome improvement revolves around the healthcare researchers engaging with the appropriate data. According to Melnyk and Fineout-Overholt (2015), EPB research involves internal and external data that provides more insight into handling a particular problem within the medical realm. Hence, the main internal evidence sources that Melnyk and Fineout-Overholt (2015, p.227) provide include quality management. For example, in the case of managing high blood pressure in our healthcare organization, the EBP researchers can evaluate the effectiveness of CPAP therapy. In addition, finance is another evidence source where healthcare can assess the cost they incur after integrating CPAP therapy. Hence, these kinds of data may be mostly found in the billing department, where reducing high blood pressure patients paying for services will prove that the CPAP therapy is working as intended by the EBP researchers.

Human resources also provide internal evidence on the patient-nurse ratio to enable the healthcare management to engage enough staff to attend to their target population. Such data can help the healthcare management make informed decisions on whether they need to employ more nurses to reduce congestion which may impact the nurses’ and patients’ well-being Melnyk and  Fineout-Overholt (2015, p.228). Clinical systems mainly align with data storage. In contrast, most healthcare systems have integrated technology such as cloud data storage to help store data in bulk without interfering with the healthcare system. Electronic health records are also part of internal evidence sources that allow interoperability, thus promoting EBP research. Lastly, the engagement of administrative data like routine operations is essential since it helps a healthcare organization assess if it is still on the right track in achieving its goals.

Conclusion

Evidence-based practice (EBP) in healthcare is an essential factor that healthcare organizations need to consider when improving their services. However, there is a need to let the healthcare providers learn more about the EBP since it prevents instances of resistance once the healthcare organization wants to engage in a new change. Therefore, there will be a need to engage these staff in workshops and training to help them gain more knowledge concerning EBP. In addition, during the project implementation process, the project team needs to engage all stakeholders to ensure that they align with the change process.
