Week 11 Assignment 1: Lab Assignment-Ethical Concerns

The selected scenario is about a 10yo who has been hospitalized following a high fall in the State Fair. The client sustained multiple internal injuries, necessitating various treatment interventions, including surgery and blood transfusions. The parents are Jehova’s Witness followers and are against blood transfusion. They prefer resuscitating the client with fluid. The parent requests the Emergency Department Nurse Practitioner in charge of the surgery to sign a waiver of liability to the medical facility in case the child loses his life.

In diagnosing the client nurse practitioner requires health assessment information. First, the past medical conditions of the client are required. The risk of sustaining internal injuries after falling is high among individuals with the severe liver disorder (cirrhosis). Flores et al. (2017) reported that cirrhosis poses an increased risk of bleeding and thrombosis and bleeding among people diagnosed with chronic liver disease. Additionally, information about the history of disorders that compromise blood clotting among family members should be collected. The risk of sustaining internal injuries after fall is a high among individuals with a family history of inherited disorders that compromise blood clotting ability such as hemophilia or Willebrand’s disease. Hereditary bleeding disorders are associated with deficiency of specific clotting proteins, resulting in bleeding episodes (Knöbl, 2018). Furthermore, information concerning the client’s current medication is required during diagnosis. The risk of sustaining internal injury and bleeding is high among individuals using blood thinners. Lastly, the client’s diagnosis is guided by details of the severity and type of injury sustained by the client. This information will guide the healthcare provider in conducting differential diagnoses to rule out conditions with similar clinical manifestations.

An advanced practice nurse would apply evidence-based practice guidelines and ethical considerations in addressing the presented scenario. The nurse should recommend blood transfusion for the client even if the parents are against this decision. Blood transfusion is the most appropriate intervention for patients with excessive bleeding to prevent anemia among surgical patients, which is a significant factor contributing to the high mortality rate in these patients (Agathis et al., 2020). Thus, a blood transfusion would reduce the risk of anemia and death in this patient. Additionally, the nurse should adhere to the ethical principle of beneficence, which holds that healthcare providers should focus on benefiting their patients through provided treatment. Therefore, the client would prevent the client from becoming anemic or dying through blood transfusion.

As an advanced practice nurse, you will run into situations where a patient’s wishes about his or her health conflict with evidence, your own experience, or a family’s wishes. This may create an ethical dilemma. What do you do when these situations occur?

In this Lab Assignment, you will explore evidence-based practice guidelines and ethical considerations for specific scenarios.

To Prepare
Review the scenarios provided by your instructor for this week’s Assignment. Please see the “Course Announcements” section of the classroom for your scenarios.

Based on the scenarios provided:
Select one scenario, and reflect on the material presented throughout this course.
What necessary information would need to be obtained about the patient through health assessments and diagnostic tests?
Consider how you would respond as an advanced practice nurse. Review evidence-based practice guidelines and ethical considerations applicable to the scenarios you selected.
The Lab Assignment
Write a detailed one-page narrative (not a formal paper) explaining the health assessment information required for a diagnosis of your selected patient (include the scenario number). Explain how you would respond to the scenario as an advanced practice nurse using evidence-based practice guidelines and applying ethical considerations. Justify your response using at least three different references from current evidence-based literature.

By Day 6 of Week 11
Submit your Assignment.

Submission and Grading Information
Throughout this course, you have explored a wide range of health assessments and abnormal examination findings. Although you have predominantly focused on the procedural aspects of health assessment, this week, you will focus on ethical considerations that should be taken into account when advising patients or their families.

This week, you will consider how evidence-based practice guidelines and ethical considerations factor into health assessments. You will also evaluate health assessment concepts related to medical religious beliefs and infertility complications in oncology patients.

Learning Objectives
Students will:

Apply evidence-based practice guidelines to make an informed healthcare decision
Apply ethical considerations to a health assessment response
Apply concepts, theories, and principles relating to sports physicals and well-child and well-woman examinations
Identify concepts, theories, and principles related to advanced health assessment

This is the scenario I chose below…
*A 10 year old is admitted to the hospital after a high fall in the State Fair. He has multiple internal injuries requiring surgery and possible blood transfusions. The parents who are Jehova Witness are refusing the transfusion and prefer to continue resuscitation with fluid. The parents offer you to sign a waiver of liability to the hospital in case of the child death. You are the Emergency Department Nurse Practitioner coordinating the surgery. What’s your response?
